Preparing For Your Appointment:

New Patient Registration Forms

Once you’ve scheduled your appointment, you have the option of having your registration forms emailed and or mailed to you prior to your appointment. To speed up the wait time when you arrive for your appointment, you can also fill out the registration online!

What To Bring

At your appointment, make sure you bring your government issued photo-ID as well as your insurance card (if any). Our front desk staff will scan them into your chart. Let our staff know if you are switching insurances or need to make updates to your information such as address, phone number, email, etc.

During Your Appointment

Check in at our front desk and our friendly staff will help your sign in and get you ready to see your dentist. Your insurance (if any) will be verified prior to your appointment and payments will be collected at your appointment if necessary. An initial photo will also be taken to be kept in your records with respect to HIPAA Guidelines. If you have not done so, you will be required to fill out new patient registration forms.

Once you’re finished with necessary paper work, our dental assistant or hygienist will take you back to take your x-rays. At Pitman Dental, we offer our patients proven techniques and cutting edge technology like digital radiography and 3D cone beam for low dose-comprehensive diagnosis. Your x-rays will help Dr. Hung Luong provide proper diagnosis to provide you with a treatment plan

When your appointment is complete, your dentist may have you schedule for another visit or to come back for a routine check-up and cleaning. Our front desk staff may also collect any payments if necessary.
